We are a one-doctor small practice and will make every effort to ensure that we are in network before you come in to see us.
**There are still times when a patient may have a plan that considers us out of network.
**For example, we are in network with Cigna, BUT there are still some plans within the Cigna network that we are considered out of network.
**This is frustrating for you and for us. Especially when it takes hours of phone time to speak with a human to determine this status.
**Please educate yourself on your insurance coverage. We encourage you to call on your behalf to make sure that your plan will cover any medical visit that you make.
Never assume your plan will cover all of your health care needs.
**In addition, we can NOT guarantee that your insurance will cover your office visit even though we try in good faith to make sure this is the case.
-We are in network with Blue Cross Blue Shield, Cigna, Aetna and Medicare.
-We are in network with Moda, but only Beacon and Connexus plans.
-We are in network with some PacificSource plans. Please call your insurance to confirm coverage.
-We love to see Providence patients but are not in network with Providence. This includes Providence medicare plans. We can bill Providence insurance, BUT---You will likely pay more out of pocket for an out-of-network provider. Please call your insurance first to confirm your coverage.
-We collect copays at the time of service because our agreement with your insurance plan requires this.
-If you are meeting a deductible or are using insurance out of network, $125 will be collected at the time of a new office visit or $75 at a follow up visit. This amount will be credited toward the final bill once the claim has been processed by your insurance. This amount includes the amount of the copay.
We do not accept and bill workman's compensation or any insurance claim injury.
