If, for any reason, you cannot keep a scheduled appointment or will be delayed, please call as soon as possible. We
like to stay on a strict schedule and you may be rescheduled if you are more than 10 minutes late. In
addition, a $50 charge will be billed if you do not show up for your scheduled appointment.
Insurance
Our office accepts a variety of HMOs, PPOs, and other health plans.
Some plans we ARE in network with (but not limited to):
- Aetna
- Cigna
- Blue Cross Blue Shield
- Moda (limited plans-call your insurance to make confirm your coverage)
- Health Net
- PacificSource
- Medicare
We are currently not a Providence Provider. We will accept and bill Providence, but are considered "out of Network". This will likely result in a higher out of network fee paid by the patient. Please call your insurer to discuss your plan and the limits of your deductible and financial responsibility.
